INSURANCE 

Country Creek Pediatrics accepts most Insurance Carriers. Verify coverage with your Insurance Carrier prior to scheduling an appointment at Country Creek Pediatrics to be sure we are a listed Provider. Dr. Samuels must be listed as the Provider with your insurance plan.

Please remember to bring your photo ID and Insurance Card with you at each visit. All co-payments are to be paid in full at the time of service by the accompanying Parent/Legal Guardian. Additionally, any processed insurance deductibles should be settled in full at the time of service.

We accept various payment methods, including Visa, Mastercard, Discover, American Express, HSA cards, and cash. Your cooperation helps us maintain a smooth and efficient billing process.

  Please note that some Insurance Carriers require Prior Authorization for referrals to a Specialist. You must inform our office of the need for Prior Authorization at least 3 days in advance.
